To request this information online, you will need:

An Active Email Address:
You can use your personal or school email address.

Student ID:
You will need your University of Miami school-issued student ID number. This can be found on your student ID card or your billing statement.

Social Security Number:
When we provide services to you, we may collect information such as the last four digits of your SSN, demographic, payroll, banking (for ACH purposes) and insurance data.

Eligibility Requirements:
In order to acquire insurance information and coverage, you must meet eligibility and attendance requirements set by your school. The insurance company (UnitedHealthcare) and/or Benefits Outsource, Inc. maintains the right to investigate student status and attendance records to verify that the policy eligibility requirements have been met. If the Company (UnitedHealthcare) discovers that the policy eligibility requirements have not been met, you will not be issued coverage. If you already have coverage, it will be terminated.

You may withdraw from this process at any time, but your information will be lost if you navigate away or close the window before you complete all the required fields and click the “submit” button. Incomplete applications will not be processed. You will receive an email to confirm that your information is being reviewed.

When enrolling dependents on dental and/or vision plan, additional documentation is required to verify dependents’ eligibility.
